We are moving away from paper forms, to be more efficient, and be more friendly to the environment.  When you make an appointment we will send you a link so you can pre-fill these forms in the comfort of your own home ahead of your appointment. Alternatively you can arrive 15mins earlier than your appointment time to fill them out.

Payment Methods

We accept a variety of payment options including cash, cheque, and all major credit cards. We also provide HICAPS facilities to allow for on-the-spot claims from your health fund.
